a. Notify Police, call for ambulance if needed, 
firemen if needed 
b. Call US, WE MUST BE NOTIFIED WITHIN 24 HOURS 
c. Fill out an accident form located in the back of 
the notebook 
d. The state accident form must be filled out and 
filed with the APPROPRIATE authorities
